Out of the box approval workflow sends duplicate emails

Situation

Customer complains about duplicate emails from out of the box SharePoint approval workflow. After a lot of research in Google, SharePoint log and etc I decided to analyze this two emails headers and found something interesting:

So second email has one more line. So, what's up?

Letter came to the server smtp1.xx.yy (Postfix) with ESMTP id 5816F1CC55 and was delivered for user. Then letter was resend to another server mail.xx.yy (xxx.yyy.21.73) with Microsoft SMTP Server and delivered for second time!

So there is a problem with smtp1.xx.yy (Postfix) server and not with a SharePoint approval workflow!

So, if you receive a duplicate mails from SharePoint standard workflows, the problay may be in mail server configuration.
